Armstrong Produce Honolulu (O'ahu) Benefits Summary

1. Health Insurance with Hawaii Medical Services Association (HMSA): Includes Medical, Drug, & Vision. Waiting period applies.

2. Dental Insurance with MetLife Dental. Waiting period applies.

3. Group Term Life Insurance: Company paid term life insurance equal to 1 ½ times annual salary rounded to nearest thousand. Also includes Accidental Death & Dismember Provision. Three month waiting period.

4. Voluntary Supplemental Life Insurance for employee, spouse and dependents, with AD&D provision option. Three month waiting period.

5. Profit Sharing/401(k) Plan with Fidelity Investments. Current plan includes a discretionary company match of 50 cents to each dollar contributed up to a maximum 6% of compensation. Three month waiting period.

6. Paid Vacations and Sick Leave available. Waiting period applies.

7. Holidays: Company observes 6 holidays. Employees are eligible for two additional Floating Holiday. Waiting period applies.
